By checking the box and clicking “Submit”  I agree to the following:

• The information submitted by me is true and correct to the best of my knowledge and that the information will be used by the Illinois Department of Public Health to investigate the nursing home named herein.
• An additional copy of the information in the submission will be forwarded to the Law Offices of Barry G. Doyle, P.C., 4709 West Golf Road, Suite 1140, Skokie, IL 60076, phone (312) 263-1080. I agree that representatives from the law firm may review the information contained in the submission and may contact me thereafter for the purposes of marketing legal services to me for the nursing home resident named in the submission.  I further agree that additional contact may be made with me by way of e-mail.
• I understand that because the information contained in the submission will be provided to the Illinois Department of Public, that information is not considered confidential and will not be covered by the attorney-client privilege.
• I understand that the Law Offices of Barry G. Doyle, P.C. is not obligated to undertake representation of the nursing home resident named herein and that without there being an attorney-client representation agreement signed by a representative of the law firm, the law firm will not be taking any action on behalf of the nursing home resident named herein.
• In the absence of direct communication between myself and representative of the law firm, no attorney-client relationship is formed.
• I agree to the terms and conditions of the Privacy Policy.
